Helpful Information from the Nurse GRADE 6 PHYSICAL EXAMINATION REQUIREMENT - Children entering Grade 6 in September 2006 will
be receiving physical examination forms soon. When you receive this form
please complete the attached statement of preference sheet and return it to
the school nurse. A physical examination by a physician is required in 6th
grade, and physicals done after June 1, 2004 are acceptable. Be sure your physician fills out the Blue Health Assessment
Form in its entirety, including the immunization section. It is recommended that you check with
your physician concerning Hepatitis B vaccine, measles vaccine, and proof of
immunity to chicken pox, which are required for 7th grade entry. IMPORTANT! - GRADE 7 IMMUNIZATION
REQUIREMENTS - In
accordance with Connecticut State Law, each student entering 7th
grade must show proof of the following: ¨
Second immunization to measles ¨
At least 1 dose of Hepatitis B vaccine or blood work showing prior
infection with Hepatitis B ¨
Proof of immunity to chicken pox. This may be in the
form of age appropriate immunization or
a blood test showing past infection or
a statement from your
physician indicating that your child has had chicken pox. Students will be excluded from 7th grade until
the school nurse receives the required documentation. Notices have been sent
to 6th grade parents whose children are missing required
immunizations for 7th grade. FROM THE SCHOOL NURSE - Here
are some procedures, if done during the summer, which can make the next
school year run more smoothly for all of us. All students who will require
limited physical education (gym excuse) for any reason – e.g., chronic
medical conditions, orthopedic problems, etc., should come to the health room
now for a form to be filled out and signed by a physician, and
returned to the school nurse on the first day of school in August, 2006. The
restricted gym excuse must be updated yearly. Also, any student who will be taking medication during
school hours of the next school year must follow the same procedure. Students
who will be participating in high school sports will require a physical
either by their private physician or the school doctor. High school sports
physicals will be done on Wednesday, August 16, 2006 and Wednesday, August
23, 2006 from 8:30 a.m. – 11:00 a.m. |
